It is University policy that all new staff and visitors complete a Staff Induction Checklist form.
This form is available on the Safety and Health website and from the UWA Safety and Health Office.
Two versions of the Staff Induction Checklist form are available, the PDF version and the hypertext version. The hypertext version is provided as an online aid to induction, by allowing all policies, procedures and referenced locations to be viewed during the induction session.
All signed forms should be collected by the supervisor and returned to the Administration Office for filing.
The induction for professional staff should be carried out by the direct supervisor.
The Head of School or a delegated senior staff member should carry out academic staff inductions.
